Faculty Reserve Request Forms

Important Note

All forms must include a designation of copyright status and a complete bibliographic citation. Forms submitted without these will be returned to the instructor.

Online Form

• The online form can be used for all requests.
• The completed form may be submitted electronically for requests that do not require the instructor to submit photocopy or personal copies.
• The completed form may be printed and submitted via campus mail or directly to the Case or Cooley Circulation Desks together with required photocopies or personal copies.

PDF Form

• The pdf form can be used for all requests.
• Print the form and submit it via campus mail or directly to the Case or Cooley Circulation Desks together with any required photocopy or personal copies.
• Print or photocopy additional copies of the second page as needed.
